Job Title: Geriatric Psychiatry Social Worker

Fraser Health is seeking a skilled Geriatric Psychiatry Social Worker to join our multidisciplinary team. As a key member of our team, you will provide assessment, treatment, consultation, and follow-up services to mentally ill elderly clients.

Responsibilities:
  • Provide clinical social work services to elderly clients and their caregivers, including comprehensive psycho-social assessments and treatment planning.
  • Carry out treatment plans and interventions, including monitoring the effects of psychotropic medication and providing counseling and crisis intervention.
  • Provide consultation, support, and informal education to families and other informal caregivers of the clients.
  • Refer clients to medical, social, and psychiatric services as necessary and stay knowledgeable of such resources.
  • Collaborate with members of the interdisciplinary team and coordinate work with them in areas such as attendance at team meetings and conferences.
  • Develop care and treatment plans for shared clients in cooperation with care facility staff, physicians, and Home Health Care.
  • Act as an advocate on behalf of clients and their families.
  • Maintain clinical and statistical documentation according to established policies and procedures.
  • Provide consultation to referral agencies, care facilities, and caregivers on improved care of the elderly with mental illness and represent the team on local committees and advisory boards as required.
  • Participate in Mental Health and Addictions Services training and planning activities as required.
  • Provide community and agency education on issues affecting the mentally ill elderly and on the role of the Geriatric Psychiatry Outreach team.
Qualifications:
  • Master's of Social Work (MSW) from an accredited school of Social Work.
  • Three (3) year's recent related experience with Geriatric Psychiatry clients, or an equivalent combination of social work education, training, and experience.
  • Current full registration with the BC College of Social Workers.
  • Valid Class V BC Driver's license and possession of a personal vehicle.
