About the Role

We are seeking a dynamic and passionate Registered Practical Nurse (RPN) to join our team in Hamilton. As an RPN with SE Health, you will have the opportunity to provide holistic nursing care to clients and their families in their homes and communities.

Key Responsibilities
  • Complete comprehensive client assessments and develop care plans based on best evidence and practices.
  • Implement and evaluate care based on client's unique needs and work in collaboration with other care team members.
  • Support clients through care transitions by connecting them with community support services.
  • Work as a primary provider for a client caseload and be accountable for their care.
Requirements
  • Canadian Nurses Association (CNA) specialty certification an asset.
  • Ability to be flexible and adaptable with excellent organizational skills.
  • Demonstrated appreciation for Quality Improvement processes.
  • Availability to work days, evenings, nights, and every other weekend.
  • Strong computer skills in MS Word, MS Excel, and Email.
