About the Opportunity

We are seeking a skilled Social Worker to join our interdisciplinary healthcare team at Nova Scotia Health and IWK Health. As a Health Care Social Worker, you will play a vital role in providing collaborative, holistic care to patients across the primary to quaternary care continuum.

Reporting to the Health Services Manager, you will be responsible for building relationships with patients and their families, assessing their psychosocial, emotional, and spiritual needs, and developing personalized care plans to address these needs.

As a member of our team, you will have the opportunity to work with a diverse range of patients and families, and to contribute to the development of innovative care solutions that improve patient outcomes.

We are looking for a candidate with a strong background in social work, excellent communication skills, and the ability to work effectively in a fast-paced healthcare environment.

About You
  • Bachelor's degree in Social Work from a recognized university
  • Current registration with the Nova Scotia College of Social Workers
  • 2 years' post BSW related clinical experience required
  • Previous experience working in community-based settings preferred
  • Demonstrated ability to use a client-/family-centered approach
  • Ability to complete biopsychosocial assessments
  • Excellent communication skills
  • Demonstrated ability to assist patients and families with navigation of local community resources and agencies
  • Travel to various sites may be required
  • Competencies in other languages an asset, French preferred
Hours and Compensation

This is a permanent, full-time position with 75 hours bi-weekly. The hourly rate is $42.89.
