Job Summary

We are seeking a highly skilled and experienced Site Access Coordinator to join our team at Fraser Health Authority. As a key member of our healthcare operations team, you will play a critical role in ensuring the efficient and timely placement of patients across our acute care sites.

Key Responsibilities
  • Coordinate patient placement activities, assessing current and projected capacity, and prioritizing critical issues requiring immediate action.
  • Consult with the Patient Transfer Network, community resources, and other stakeholders to identify appropriate care for patients and develop effective bed utilization strategies.
  • Supervise and coordinate clinical staff, providing work direction and making clinical and operational decisions as needed.
  • Act as a clinical resource for nursing staff, providing guidance on patient placement and flow, and bed utilization.
  • Develop and recommend changes to patient access and flow policies and procedures to optimize timely bed access and patient placement.
Requirements
  • Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
  • Five (5) years' recent, related clinical experience in an acute care, multidisciplinary facility, including nursing leadership experience.
  • Ability to apply continuous quality improvement tools and techniques in problem-solving.
  • Ability to supervise in a multidisciplinary setting.
  • Ability to prepare and present educational material.
