About the Opportunity

The Continuing Care Coordinator plays a vital role in the assessment, care planning, and authorization of services for clients referred to Continuing Care. This position requires strong assessment and interviewing skills to identify clients' goals, unmet needs, and resources. The Coordinator works collaboratively with clients, families, caregivers, and healthcare teams to develop and implement care plans that maximize available services and resources in the community.

The ideal candidate will possess a Bachelor's Degree in Nursing, Social Work, Occupational Therapy, Physiotherapy, or Nutritional Studies, and current registration with the applicable Nova Scotia Professional Regulatory body. A minimum of three years' experience in a community or long-term care setting is required, along with knowledge of community agencies and resources. Excellent assessment, interviewing, and negotiation skills are essential, as well as the ability to work independently and collaboratively in a busy multidisciplinary environment.

The Continuing Care Coordinator will be responsible for:

  • Assessing clients' needs and developing care plans that maximize available services and resources in the community
  • Authorizing services and making referrals to other professional and volunteer resources
  • Facilitating discharge to return to the community as required
  • Maintaining ongoing communication with clients, families, caregivers, and healthcare teams

This is a permanent full-time position, with a 70-hour bi-weekly schedule. Shifts may include days, evenings, weekends, and holidays.
