Continuing Care Coordinator

Nova Scotia Health is seeking a skilled Care Coordinator to join our team. As a Care Coordinator, you will play a vital role in assessing, planning, and coordinating care for clients referred to Continuing Care.
About the OpportunityThe Care Coordinator is responsible for:
- Assessing clients' needs and developing care plans
- Authorizing services and making referrals to other resources
- Facilitating discharge to return to the community as required
- Maintaining ongoing communication with clients, family, caregivers, and healthcare teams
The Care Coordinator will work in the community and hospital, and with clients in long-term care facilities. They will be fiscally responsible for the care/services/equipment they authorize within available Department of Seniors and Long Term Care policies and programs.
About YouWe are looking for a candidate with:
- A Bachelor's Degree in Nursing, Social Work, Occupational Therapy, Physiotherapy, or Nutritional Studies
- Current and ongoing registration with the applicable Nova Scotia Professional Regulatory body
- Minimum three years' experience with the professional regulatory body
- Current BLS-C and Emergency First Aid certification
- Demonstrated knowledge and/or experience in a community or long-term care setting
- Knowledge and/or experience in dedicated case management or discharge planning
- Competent with computer use, keyboarding, and able to conduct assessments using a laptop
- Knowledge of community agencies and resources
- Excellent assessment, interviewing, and negotiation skills
- Excellent verbal and written communication skills
- Ability to work independently and co-operatively in a busy multidisciplinary situation
- Ability to multi-task, prioritize workload, and manage time efficiently
- Valid driver's license and access to a reliable vehicle
- Competencies in other languages preferred; French an asset

Hours- Permanent Full Time; 70 Hours Bi-weekly
- Shifts may include days, evenings, weekends, and holidays
The successful candidate will receive a competitive hourly rate of $46.75 and may be eligible for our benefits package, which includes health, dental, travel, long-term disability, and life insurance coverage, as well as a defined benefit pension plan.
