Healthcare Team Leader - Mississauga - Full Time

As a Healthcare Team Leader within our CBI Home Health organization, you will oversee various initiatives within our Personal Support Services.

Your role involves guiding a team of direct care professionals to ensure that the delivery of clinical services meets or surpasses the standards established by CBI Home Health's external partners.

As a Regulated Health Professional (RPN), you will create and tailor care plans to address the unique needs of each patient, while supervising both regulated and unregulated staff in the execution of services that align with their professional scope, emphasizing service and clinical excellence.

PRIMARY RESPONSIBILITIES
As a Healthcare Team Leader, you will:
  • Respond to patient referrals promptly.
  • Conduct thorough care assessments in a timely manner, utilizing established tools and processes.
  • Document all necessary information related to client status and care planning.
  • Ensure the safety of patients and care providers by accurately assessing risks.
  • Confirm that care plans are tailored to individual patient needs and that care providers possess the requisite knowledge and skills.
  • Reassess patients during interim visits to ensure the ongoing appropriateness of service plans.
  • Manage the investigation, resolution, and reporting of all client-related concerns.
  • Promote service excellence through effective onboarding, mentoring, coaching, and performance management of field staff.
  • Ensure all team members are aligned with their key responsibilities and performance expectations.
  • Actively participate in achieving utilization, retention, and engagement goals for field staff.
  • Address identified training needs of staff with actionable plans to close gaps and minimize risks.
  • Ensure compliance with all internal and external reporting standards.
  • Complete all documentation accurately and in a timely manner.
  • Identify potential risks and collaborate with internal stakeholders to mitigate them.
  • Respond comprehensively to Incident Reporting.
  • Enforce policies and procedures that reflect best practices.
QUALIFICATIONS
To qualify for the role of Healthcare Team Leader, you must:
  • Hold a current registration with the College of Nurses of Ontario (CNO) as a Registered Practical Nurse.
  • Have a minimum of 2 years of supervisory experience or experience in home health service delivery.
  • Be capable of establishing and nurturing relationships with clients and care staff.
  • Possess access to a reliable vehicle and a valid driver's license, with the ability to travel as needed.
  • Demonstrate problem-solving skills that align with policies and best practices.
  • Be self-motivated and able to work independently as well as collaboratively within a team.
  • Exhibit strong verbal and written communication skills in English; proficiency in French or other languages is an asset.
  • Commit to working every second weekend.
