The Activity Instructor II (AI2) is a key member of the Prairie Mountain Health team, responsible for providing mobile community-based services for individuals with mental health problems. This role requires a strong therapeutic relationship with clients, assessing their strengths, interests, and goals to develop a personalized plan that promotes empowerment, independence, and self-responsibility.
Key Responsibilities:
- Therapeutic Relationship and Assessment: Engage with clients to develop a strong therapeutic relationship, assessing their individual strengths, interests, and goals to inform the development of a personalized plan.
- Program Development and Implementation: Develop and implement a variety of individual and group programming and supports that engage clients with their environment, promoting optimal functioning and preventing or postponing further disability.
- Collaboration and Communication: Work in collaboration with clients, their support network, and other healthcare professionals to ensure comprehensive care and support.
- Group Facilitation and Community Engagement: Facilitate group activities, develop individual and small group awareness of resources, and link clients to community agencies and resources to achieve their personal goals.
- Health Promotion and Disease Prevention: Participate in regular planning to organize and deliver programs that promote health, communicate with clients and partners to increase awareness and understanding of mental health issues, and implement programming aimed at preventing further disability.
- Crisis Management: Facilitate ongoing client education and training related to safe work practices, mediate risk and address safety concerns, and assess and intervene in crisis situations.
- Program Administration: Maintain activity program statistics, record assessments and evaluations, ensure inventory of supplies, and prepare area budget and reports.
Requirements:
- Strong therapeutic relationship skills and ability to assess client needs.
- Ability to develop and implement individual and group programming and supports.
- Excellent communication and collaboration skills.
- Ability to work in a fast-paced environment and prioritize workload.
- Knowledge of mental health issues and community resources.
