Job Title: Social Worker

Job Summary:

We are seeking a skilled Social Worker to join our team at Fraser Health. As a Social Worker, you will play a vital role in providing clinical and consultative services to patients and families, conducting assessments, counseling, and planning for patient discharge.

Key Responsibilities:

  • Conduct social work assessments to identify patient and family coping abilities and anticipated needs upon discharge.
  • Provide clinical counseling services to patients and families regarding identified problems, personal concerns, and treatment objectives.
  • Consult and collaborate with interdisciplinary teams regarding patient care issues, resources, and discharge planning.
  • Provide consultative and direct services to patients, families, and staff regarding community resources and access to these resources.
  • Participate in department or program-sponsored research through literature searches, administering surveys, and analyzing data.
  • Provide in-service education to staff through presentations and workshops.
  • Participate in program and community resource development activities by working with community service agencies and the hospital.

Qualifications:

  • Bachelor's degree in Social Work from an approved school of Social Work.
  • One year recent related clinical social work experience in a healthcare setting.
  • Current full registration with the BC College of Social Workers.

Skills and Abilities:

  • Knowledge and understanding of psycho-emotional and social issues related to illness/injury/disease and current social work treatments.
  • Counseling skills and clinical practice models, including brief solution-focused therapeutic intervention, crisis intervention, and resource counseling.
  • Legislation/acts applicable to social work within the healthcare environment.
  • Effective communication and collaboration skills.
