In this community nursing position, you will be responsible for scheduled visits to provide care for patients in various environments, with the assistance of Registered Nurses for more complex cases. Key Responsibilities:

  • Conduct scheduled visits in the community to address patient needs within your scope of practice
  • Deliver ethical and safe nursing care
  • Utilize appropriate assessment tools and techniques to evaluate patients' individual needs
  • Implement and continuously monitor care plans, making adjustments as necessary
  • Engage patients through a collaborative approach, fostering rapport and trust
  • Maintain and update professional patient records related to encounters and care plans

Qualifications:

  • Registration with the CNO as a Registered Practical Nurse in good standing
  • Experience in community care is advantageous
  • Reliability and flexibility are essential
  • Willingness to travel to and from patient residences within a designated area
  • Experience working with vulnerable populations
  • A passion for making a positive impact in the community
  • Willingness to participate in on-call coverage to ensure patient care during critical times, including after hours
