In this vital community nursing position, you will serve as a Registered Practical Nurse (RPN), conducting scheduled visits to provide care for patients in diverse environments, supported by Registered Nurses for more intricate cases. Key Responsibilities:

  • Conduct scheduled visits in the community to address patient needs within your professional scope.
  • Deliver ethical and safe nursing care.
  • Utilize appropriate assessment tools and techniques to evaluate patients' individual needs.
  • Implement and continuously monitor care plans, making adjustments as necessary.
  • Engage patients through a collaborative approach, fostering rapport and trust.
  • Maintain and update professional patient records related to encounters and care plans.

Qualifications:

  • Registration with the CNO as a Registered Practical Nurse in good standing.
  • Experience in community care is an asset.
  • Reliability and flexibility are essential.
  • Willingness to travel to and from patient residences within a designated area.
  • Experience working with vulnerable populations.
  • A passion for making a positive impact in your community.
  • Willingness to participate in on-call coverage to ensure patient care during times of need, including after hours.
