Key Responsibilities:

  • Conduct scheduled community visits to address patient needs within your professional scope.
  • Deliver ethical and safe nursing care.
  • Utilize appropriate assessment tools to evaluate individual patient requirements.

Qualifications:

  • Registration with the CNO as a Registered Practical Nurse in good standing.

  • Willingness to travel to patient residences within a designated area.
  • Experience working with vulnerable populations.
  • A passion for making a positive impact in the community.
  • Availability for on-call coverage to ensure patient care continuity, including after hours.
