Key Responsibilities:
- Conduct scheduled community visits to address patient needs within your professional scope.
- Deliver ethical and safe nursing care.
- Utilize appropriate assessment tools to evaluate individual patient requirements.
- Implement and continuously review care plans, making adjustments as necessary.
- Foster patient engagement through a collaborative approach, building trust and rapport.

Qualifications:
- Registration with the CNO as a Registered Practical Nurse in good standing.
- A valid driver's license and access to a vehicle are required.
- Experience in community care is advantageous.
- Demonstrated reliability and flexibility.
- Willingness to travel to patient residences within a designated area.
- Experience working with vulnerable populations.
- A passion for making a positive impact in the community.
- Availability for on-call coverage to ensure patient care continuity, including after hours.
