Healthcare Client Coordinator

Patient Care Coordinator at BC Mental Health & Substance Use Services
Key Responsibilities:
- Oversee client care initiatives and lead the development of innovative service delivery strategies.
- Work collaboratively with a multidisciplinary team to set appropriate care objectives and personalized care plans.
- Support the implementation of care standards, guidelines, and protocols.
- Participate in client care assessments and admission discussions as part of a collaborative team.
- Evaluate client progress and offer guidance to healthcare professionals.
Required Qualifications:
- Graduation from an accredited Nursing School with active registration.
- Completion of a post-basic specialty nursing program along with relevant experience.
- Possession of a valid Driver's License and CPR Certification.
Essential Skills & Knowledge:
- Understanding of mental health disorders, addiction issues, and recovery methodologies.
- Capability to provide leadership, perform assessments, and develop treatment strategies.
- Familiarity with the Truth & Reconciliation Commission's Calls to Action.
- Dedication to supporting reconciliation efforts in Canada.
