Patient Care Coordinator at BC Mental Health & Substance Use Services

Key Responsibilities:

  • Oversee client care initiatives and lead the development of innovative service delivery strategies.
  • Work collaboratively with a multidisciplinary team to set appropriate care objectives and personalized care plans.
  • Support the implementation of care standards, guidelines, and protocols.
  • Participate in client care assessments and admission discussions as part of a collaborative team.
  • Evaluate client progress and offer guidance to healthcare professionals.

Required Qualifications:

  • Graduation from an accredited Nursing School with active registration.
  • Completion of a post-basic specialty nursing program along with relevant experience.
  • Possession of a valid Driver's License and CPR Certification.

Essential Skills & Knowledge:

  • Understanding of mental health disorders, addiction issues, and recovery methodologies.
  • Capability to provide leadership, perform assessments, and develop treatment strategies.
  • Familiarity with the Truth & Reconciliation Commission's Calls to Action.
  • Dedication to supporting reconciliation efforts in Canada.
