We are seeking a highly skilled and compassionate Registered Nurse/Registered Psychiatric Nurse to join our team at Fraser Health. As a Community Care Specialist, you will play a vital role in providing exceptional client care and support to individuals with complex or chronic illnesses, chronic pain, mental health, and/or concurrent disorders.
Key Responsibilities- Establish and maintain therapeutic relationships with clients, ensuring their autonomy and dignity in decision-making and care planning.
- Conduct comprehensive client assessments, develop and implement individualized care plans, and provide clinical interventions as needed.
- Collaborate with interdisciplinary teams, primary care providers, and community resources to ensure seamless care coordination and transitions.
- Provide health counseling, education, and self-management support to clients, promoting their overall health and well-being.
- Maintain accurate and detailed clinical records, including intake screens, client assessments, and progress notes.
- Participate in quality improvement initiatives, research opportunities, and education of nursing staff and students.
- Current practicing registration as a Registered Nurse and/or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
- One year recent related experience providing treatment services to individuals with complex or chronic illness, chronic pain, mental health, and/or concurrent disorders.
- Valid BC Driver's license and access to a personal vehicle for business-related purposes.
- Demonstrated knowledge, skills, and competence in areas such as individuals living with complex and/or chronic illness, chronic pain, mental illness, or substance use treatment.
- Ability to communicate effectively, both verbally and in writing, and integrate and evaluate pertinent data from multiple sources to problem-solve effectively.
- Knowledge of broad health care services, community resources, and their role in providing a continuum of care.
