Key Responsibilities- Provide primary health care services to assigned populations, including health assessment, health promotion, and disease prevention.
- Collaborate with Primary Health Care Teams, physicians, and health care provider team members to deliver comprehensive care.
- Develop and deliver formal education programs to promote health and wellness.
- Use advanced facilitation, decision-making, and problem-solving skills to provide effective care.
- Work independently and as a member of a multi-disciplinary team to achieve shared goals.
- Master of Nursing - Nurse Practitioner (MN-NP) degree.
- Three (3) years nursing experience, with experience in a Primary Health Care setting preferred.
- Valid Class 5 driver's license.
- Certification in recognized Primary Care NP equivalent program is acceptable.
- Advanced facilitation skills.
- Variety of computer programs.
- Basic communication skills to provide/obtain verbal and/or written opinions/advice.
