Role Overview:
As a Transitional Support Professional, you will work part-time to assist individuals with daily living activities and facilitate personal growth as outlined in individualized support plans.
Key Responsibilities:
1. Collaborate with team members, families, advocates, and community partners to ensure comprehensive support.
2. Provide reports on clinical interventions as part of your clinical duties.
3. Liaise with clinical professionals, gather relevant data, and implement prescribed programs and treatments to meet the clinical, behavioral, and mental health needs of those we support.
4. Complete administrative tasks as assigned.
5. Utilize applied behavior management techniques to support individuals effectively, especially in crisis situations, employing verbal de-escalation and physical interventions as necessary.
Qualifications:
- Developmental Service Worker Diploma or Apprenticeship, Child and Youth Care Diploma (CYC), or related education preferred.
- Advanced proficiency in computer applications (MS Word and Excel).
Work Schedule:
Flexible shifts including days, afternoons, evenings, alternating weekends, and occasional overnight shifts.
