Support Individuals with Acquired Brain Injuries

Traverse Independence is a leading provider of services for individuals with acquired brain injuries (ABI). We are seeking a skilled Community Facilitator to join our team and provide support to clients in their homes or community settings across Wellington County.

As a Community Facilitator, you will work closely with clients to develop personalized support plans, build trusted relationships, and empower them to achieve their goals. You will also collaborate with our multidisciplinary team, including regulated staff such as ABI specialists, occupational therapists, and behaviour therapists.

We offer a comprehensive training program for individuals with no prior experience working with ABI clients. Our team is committed to providing a supportive and inclusive work environment, and we are eager to welcome a dedicated and compassionate individual to our team.

Requirements:
  • Community College Diploma in Social Services, Developmental Services, Health Care, or related discipline.
  • Demonstrated experience in behaviour management skills is an asset.
  • Strong organizational skills, interpersonal skills, and communication skills.
  • Ability to work as a member of a multidisciplinary team.
  • Valid driver's license and access to reliable vehicle with proof of $1,000,000 liability insurance and driver's abstract acceptable to Traverse Independence.
