About the Role: Care Navigator

We are seeking a compassionate and dedicated Care Navigator to join our team. In this role, you will play a vital part in guiding seniors and their families through the complexities of healthcare and community resources.

Responsibilities:
  • Develop personalized service and care plans in collaboration with clients and caregivers.
  • Provide expert guidance and support as clients navigate the healthcare system.
  • Connect clients and families to appropriate community resources and support services.
  • Conduct thorough assessments to identify client needs and develop tailored solutions.
  • Offer crisis intervention and monitor client well-being, ensuring their safety and stability.
Qualifications:
  • Registered Health Care Professional with a license in a relevant field (e.g., Registered Nurse, Physiotherapist, Occupational Therapist, Speech Language Pathologist, or Social Worker). A Registered Social Worker designation is highly valued.
  • Minimum of one year of experience working within the community support sector or healthcare environment.
  • Proven experience supporting frail and elderly populations, with a strong understanding of available community resources and funding options.
  • Demonstrated expertise in case management/coordination, including assessment, scheduling, implementation of services, care planning, monitoring, and evaluation processes.
  • In-depth knowledge of in-home health care issues and familiarity with the Assisted Living Services for High Risk Seniors Policy.
  • Proficiency in computer skills, including MS Word, Outlook, and PowerPoint. Experience with CIMS and the RAI-CHA is an asset.
  • Valid Ontario Driver's License and access to a reliable vehicle.
  • Current Vulnerable Police Record check completed within the last 12 months.
  • Up-to-date CPR/First Aid Certification.
  • Fluency in other languages is an asset.

On-call duties are required. This position requires full vaccination against COVID-19, with proof of vaccination provided prior to employment.
