We are seeking a skilled and compassionate Community Nurse to join our team at Saint Elizabeth Health Care. As a Community Nurse, you will play a vital role in providing high-quality patient care and support to individuals in their homes and communities.
Key Responsibilities- Conduct comprehensive client assessments and develop personalized care plans to meet their unique needs.
- Implement and evaluate care based on best evidence and practices, working collaboratively with other healthcare professionals.
- Support clients through care transitions by connecting them with community resources and services.
