We are seeking a skilled and compassionate Registered Nurse (RN) or Registered Practical Nurse (RPN) to join our team as a Community Nurse. Key Responsibilities- Develop and maintain effective care plans with respect to community health services, independently or in cooperation with the Ontario Health atHome.

- Communicate and liaise with patients, family members, and other members of the interdisciplinary care team.
- Perform other clinical duties as required.
