Registered Nurse – Various Positions (contract and permanent)
WellFort Community Health Services is a forward-thinking community health center committed to transformative care delivery through integrated services across diverse primary care models. We are seeking to fill multiple positions catering to a wide range of patient demographics, including those in new neighborhood primary care networks, shelters, and community settings. We are recruiting for various roles (both contract and permanent) and multiple full-time equivalents (FTEs). The roles may involve work across different programs and locations within the Central West Ontario Health Team area. If you are interested, please indicate your FTE availability in your application. KEY RESPONSIBILITIESDirect Patient Care
The RN is instrumental in facilitating primary care services and encouraging healthy lifestyle choices among community residents. The RN provides comprehensive care and serves as a gateway to broader health and social services, including:
- Collaborating with the healthcare team to strategize and assess programs, and initiating health education, screening initiatives, or workshops by identifying the health needs of clients, families, and community groups.
- Executing clinical responsibilities by performing nursing assessments, offering health education and counseling, gathering patient histories, conducting specific procedures in accordance with the College of Nurses of Ontario standards, and providing follow-up care.
- Engaging in the design, execution, monitoring, and evaluation of treatment, education, counseling, and health promotion services for individuals, families, and the community.
Health Promotion and Education
- Identifying trends or issues affecting the health status of priority populations and utilizing this information for planning and advocacy.
- Actively contributing to the planning, development, implementation, and evaluation of relevant health promotion and education programs in collaboration with community partners.
- Delivering health information and counseling to individuals and groups through discussions, workshops, presentations, health fairs, and written materials.
- Serving as a resource for WellFort staff and community organizations on various primary health care issues within the registered nursing scope of practice.
- Enhancing awareness of health promotion initiatives among clients, including immunizations, smoking cessation, risk assessments, and lifestyle modifications.
Advocacy and Community Engagement
- Advocating for clients to eliminate barriers to accessing health care and other services.
- Working with clients, particularly those facing access challenges, to ensure equitable treatment.
- Participating in the formulation and support of public policies that positively impact health determinants.
- Engaging with the community to bolster its capacity for independent functioning, environmental protection, and the development of healthy public policies.
- Involvement in community projects or WellFort initiatives to provide education and develop collaborative strategies.
Administrative Duties
- Maintaining precise and timely electronic documentation, including EMR, requisitions, reports, and other necessary information.
- Contributing to the development of policies and protocols related to health service delivery and professional practice.
- Staying informed about government guidelines, acts, and legislation affecting client care.
- Keeping abreast of evidence-based strategies that enhance health outcomes.
- Supervising and educating students from various disciplines, focusing on health determinants and the role of nursing.
- Managing the ordering, maintenance, and appropriate handling of medications, vaccines, and supplies.
- Participating in research opportunities as they arise.
- Performing additional duties as assigned.
- Diploma or degree in nursing from an accredited institution; a baccalaureate degree is an asset.
- Current registration with the College of Nurses of Ontario.
- Experience in registered nursing across various primary care settings, including community health.
- Familiarity with working with communities and individuals facing barriers to traditional healthcare access.
- Understanding of the determinants of health and social factors influencing health outcomes.
- Strong written and verbal communication skills tailored to meet the needs of diverse individuals and communities.
- Experience handling confidential and sensitive information.
- Excellent interpersonal skills and the ability to manage multiple tasks in a dynamic environment.
- Experience in a value-driven non-profit or social services context.
- Ability to work both independently and collaboratively within an interdisciplinary team.
- Proficiency in using computers and various software applications, including Electronic Medical Records (EMR).
- Multilingual abilities relevant to the community served are advantageous.

Compensation: $64,094 – $75,638 (pro-rated to FTE) based on experience, including health and dental benefits and pension options.
