We are seeking a skilled Community Health Nurse to join our team at PHSA. As a Community Health Nurse, you will work with clients who are incarcerated in one of BC's 10 provincial correctional facilities, providing care and support to help them transition back into the community.
Key Responsibilities:- Communicate with the healthcare interdisciplinary team to coordinate client care and ensure a smooth transition from correctional facilities to community-based services.
- Work with clients, their families, and community resources to identify and address their health and social needs.
- Develop and implement individualized care plans to support clients' transition to community-based services.
- Collaborate with healthcare providers, social workers, and other community agencies to ensure comprehensive care and support.
- Provide education and support to clients and their families on health and wellness topics.
- Graduation from an approved School of Nursing with current practicing registration as a Registered Nurse or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
- Two (2) years recent, related experience in acute or primary care or community health nursing or an equivalent combination of education and experience.
- Successful completion of Occupational First Aid II (OFA II) (or OFA III techniques for positions at Ford Mountain Correctional Centre).
