Your role will involve collaborating with the interdisciplinary team to develop transition plans for clients, ensuring they receive the necessary care and support upon release.

Key Responsibilities:

  • Develop and implement transition plans for clients, ensuring they receive the necessary care and support upon release.
  • Collaborate with the interdisciplinary team to provide comprehensive care to clients, including medical, mental health, and substance use services.
  • Communicate effectively with clients, families, and healthcare providers to ensure seamless care transitions.
  • Participate in quality improvement initiatives to enhance the delivery of care services.

Requirements:

  • Graduation from an approved School of Nursing with current practicing registration as a Registered Nurse or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
  • Two (2) years recent, related experience in acute or primary care or community health nursing or an equivalent combination of education and experience.
  • Successful completion of Occupational First Aid II (OFA II) (or OFA III techniques for positions at Ford Mountain Correctional Centre).
