We are seeking a skilled Community Health Nurse to join our team at PHSA. As a Community Health Nurse, you will work with clients who are incarcerated in one of BC's 10 provincial correctional facilities, providing them with access to healthcare treatments and support. Your role will involve collaborating with the interdisciplinary team to develop transition plans for clients, ensuring they receive the necessary care and support upon release.
Key Responsibilities:
- Develop and implement transition plans for clients, ensuring they receive the necessary care and support upon release.
- Collaborate with the interdisciplinary team to provide comprehensive care to clients, including medical, mental health, and substance use services.
- Communicate effectively with clients, families, and healthcare providers to ensure seamless care transitions.
- Participate in quality improvement initiatives to enhance the delivery of care services.
Requirements:
- Graduation from an approved School of Nursing with current practicing registration as a Registered Nurse or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
- Two (2) years recent, related experience in acute or primary care or community health nursing or an equivalent combination of education and experience.
- Successful completion of Occupational First Aid II (OFA II) (or OFA III techniques for positions at Ford Mountain Correctional Centre).
