Community Health Nurse

We are seeking a skilled Community Health Nurse to join our team at the Prince George Regional Correctional Centre. As a Community Health Nurse, you will play a vital role in providing high-quality care to clients with complex mental health and substance use challenges.
Key Responsibilities:- Develop and implement individualized care plans to meet the unique needs of clients.
- Collaborate with interdisciplinary teams to ensure seamless transitions of care.
- Provide education and support to clients and their families on health-related topics.
- Conduct assessments and provide recommendations for community-based services.
- Participate in quality improvement initiatives to enhance patient care and outcomes.
- Graduation from an approved School of Nursing with current practicing registration as a Registered Nurse or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
- Two (2) years recent, related experience in acute or primary care or community health nursing or an equivalent combination of education and experience.
- Successful completion of Occupational First Aid II (OFA II) (or OFA III techniques for positions at Ford Mountain Correctional Centre).
