Overview

Responsible and accountable to the Manager of Interprofessional practice for the provision of patient care, coordination of service delivery, education, professional/program development and quality assurance duties.
Provides comprehensive assessment and counselling of patients in the renal program, both in dialysis and in renal clinics.

Responsibilities:

1. Assesses psychosocial needs of patients and families.
2. Advocates for services to meet specific patient's needs.
3. Develops treatment plans for patients/families.
4. Assesses and counsels patients in the area of new diagnosis and chronic illness management.
5. Home visits may be required.

Education

1. Graduate of an accredited university program in Social Work.
2. Minimum Masters of Social Work required.
3. Registered with the Ontario College of Social Workers & Social Service Workers or a candidate for registration.

Qualifications

1. Recent experience with renal patients in a dialysis and/or renal clinic setting required.
2. Excellent working knowledge of resources available to patients with chronic illness.
3. Skilled in Chronic Illness management counselling and education.
4. Excellent interpersonal and communication skills.
5. Willingness to participate/lead health service councils and task groups.
6. Motivated to practice in accordance with the mission and vision of hospital/health service.
7. Good work and attendance record required.
