Clinical Social Worker Position

The Alzheimer Society Southwest Partners is seeking a skilled Clinical Social Worker to join our dedicated team. In this role, you will work closely with individuals living with dementia and their care partners to provide essential support and resources. As a Clinical Social Worker, you will be responsible for conducting psychosocial assessments, developing personalized care plans, and offering supportive counseling as needed.
Key Responsibilities:
- Conduct psychosocial assessments and develop comprehensive care plans
- Respond to client referrals and offer supportive counseling as needed
- Maintain accurate records and reports, ensuring compliance with relevant regulations
- Facilitate education and support groups, promoting community engagement and resource utilization
Requirements and Qualifications:
- Bachelor of Social Work (BSW) degree or equivalent
- Registration with the Ontario College of Social Workers and Social Service Workers (OCSWSSW)
- Minimum 3 years of experience in client services, preferably in healthcare or social services
- Strong assessment and case management skills, with ability to work independently and collaboratively
Working Conditions:
- Hybrid work environment: in-office setting and some remote work from home
- General office duties, typing, filing, occasional lifting/carrying 25lbs, reaching, bending, walking, sitting, and standing
