Social Services Professional

We are seeking an experienced Social Services Professional to join our clinical team. This role requires strong communication and interpersonal skills, with the ability to build rapport with clients and their families. You will work under the direction of the Manager of Clinical Services, providing essential support and resources to individuals living with dementia and their care partners.
Key Responsibilities:
- Conduct psychosocial assessments and develop comprehensive care plans
- Respond to client referrals and offer supportive counseling as needed
- Maintain accurate records and reports, ensuring compliance with relevant regulations
- Facilitate education and support groups, promoting community engagement and resource utilization
Requirements and Qualifications:
- Bachelor of Social Work (BSW) degree or equivalent
- Registration with the Ontario College of Social Workers and Social Service Workers (OCSWSSW)
- Minimum 3 years of experience in client services, preferably in healthcare or social services
- Strong assessment and case management skills, with ability to work independently and collaboratively
