This position is a part of the Association of the Academic Staff of the University of Alberta (AASUA).

In accordance with the Trust/Research Academic Staff Agreement, this position offers a comprehensive benefits package found on our Benefits Overview page and an annual salary range of $76,135 to $104,709. This position has an approximate end date of December 30, 2025.

**Position**

This is a new practice facilitator position for the ADAPT (A Disease Inclusive Pathway for Transitions in Care) project, a research study focused on implementing Alberta’s Home to Hospital to Home (H2H2H) Transitions Guideline and developing and implementing a co-designed standardized clinical pathway across the province. The practice facilitator will report to the principal investigator of the ADAPT Project, Dr. Sara Davison, to lead and support participating primary care clinic teams as they design and implement clinic-level changes to improve care for patients transitioning out of hospital and back to community.

**Duties**
- Serve as a key point of clinical/quality improvement (QI) contact/resource for study teams in the pre-implementation and implementation phases of the research grant.
- Oversee the implementation of ADAPT pathways across participating primary care clinics and build quality improvement capacity within these clinics and teams.
- Formally meet with each study team bi-monthly to discuss successes, barriers and needs, then advance operationalization of QI and clinic efficiency needs to support forward momentum in alignment with the broader study objectives.
- Skillfully engage multiple stakeholders, internal and external to the organization, to develop and implement effective and efficient solutions and health services.
- Support clinics to introduce measurement and metrics and extract data from their Electronic Medical Records (EMRs) and assist with the interpretation, analysis, and reporting of the data.
- Engage with ADAPT team members as required to provide expertise and training on the functionality of the various record databases.
- Lead in the creation of standardized reports at the clinic level and deliver their content to the member/clinic in a way that adds value to their practice and assists in achieving ADAPT goals and outcomes.

**Minimum Qualifications**
- Registered Nursing Degree or a Masters degree in a related healthcare field; equivalent combination of education and experience may be considered.

**Preferred Qualifications**
- 8 years experience in a healthcare related sector (5 year's experience working in primary care preferred),
- Extensive experience with project management, quality improvement and facilitation,
- Certifications in change management approaches (i.e., PROSCI, TOP training),
- Proficiency using one or more of the major electronic medical records in Alberta,
