**General Job Description**:
The Crisis Intervention Case Manager will assist clients who present, either by phone or in person, in a state of crisis. Crisis is defined as the onset of an emotional disturbance or situational distress involving a sudden breakdown of an individual’s ability to cope. The Crisis Intervention Case Manager will provide timely care to meet the needs of the client including environmental interventions and crisis stabilization, crisis support/counselling, assessment, planning, and enable the client to utilize agency or community services and resources through advocacy and collaborative consultation.
**Qualifications**:
- BSW or equivalent preferred. A combination of education and experience will be considered.
- Must be familiar with mental health, suicide intervention skills (ASIST), Solution focused therapy, psychological first aide, harm reduction, gender identity, gender-based violence, homelessness, and sexual health.
- Demonstrated knowledge of relevant legislation (e.g. the Mental Health Act; Child and Family Services Act);
- Knowledge of Indigenous cultures and French language an asset
- Must have access to a reliable personal vehicle and a current valid class ‘G’ driver’s license, and insurance to cover transportation of clients;
**Job Status**:Full Time - contract
**Salary**: per Collective Agreement

**Schedule**: 7-hour shift, Monday to Friday
**Responsibilities**:
- Crisis Support/Counseling - provides the individual and family with emotional support, practical assistance and access to a range of appropriate resources available to resolve the immediate crisis.
- Medical Intervention - is an integral part of the crisis response system. It is important to develop links between medical and non-medical service providers to ensure access to resources to resolve the crisis.
- Environmental Interventions and Crisis Stabilization - involves access to required services to stabilize the crisis and includes direct action within the individual’s community to provide supports such as arranging for money/income support, dealing with employers, planning for long/short-term housing/accommodation issues and addressing family issues.
- Review/Follow-up/Referral - provides appropriate referral to ongoing services and supports that have been mutually defined by the consumer and service provider once the crisis has dissipated.
- Monitoring and Evaluation - evaluates the achievement of goals (from the perspective of both the consumer and service provider) and consumer satisfaction.
- Information, Liaison, Advocacy and Consultation/Collaboration - provides information to the client, family/key supports and service providers regarding types of services and supports available. Works to establish partnerships among service providers to create an integrated service network, and advocates and consults on behalf of consumers and families/key supports within the service network.
- Maintain accurate client records and statistical reports as required

- To attend and participate in staff and agency meetings
