Seniors' Mental Health Mobile Community Support

A. JOB SUMMARY:
Responding to the needs of isolated Seniors impacted by the fallout of the Covid-19 Pandemic. The Mobile Community Support Worker (MCSW) will provide person-centered supports to participants who reside in various living arrangements in Elliot Lake and along the North Shore of East Algoma. The MCSW will assist to meet client needs and enable them to utilize community services and supports to achieve their optimal level of health and functioning in accordance with the program’s stated purpose and within the policies and procedures of North Shore Community Support Services, Inc.
- to provide assisted outreach supports to client(s) registered, referred by and in conjunction with the Seniors’ Mental Health Community Consultation Outreach Program.
- to offer links to community resources, formal and/or natural supports inclusive follow up (case management functions) as well as introducing social and recreational resources that can help to provide to clients a sense of belonging and community.
- to facilitate connectivity to family, friends and/or providers through telephone, virtual means and in person following Pandemic Health authority Guidelines, restrictions and safety measures.
- to provide practical, hands-on, assistance which will support clients to live independently in the community (i.e. learning new skills or refresh forgotten skills, such as planning, cleaning, shopping, healthy and safe meal preparation, safe food storage and social skills)
- to make the efforts to cultivate public awareness and support.
B. DUTIES AND RESPONSIBILITIES:
- 1. _Client Services:_
a) provide direct and indirect support for clients in the community.
b) develop with clients short and long-term goals based on client priorities.
c) review and discuss supports and outcomes of supports regularly with the Nurse of the NSCSS SMH program to problem solve.
d) provide on-going monitoring and evaluation of the client and progress and adjust accordingly.
e) co-ordinate with the Nurse of the Seniors’ Mental Health Program approaches
- to promote client medication compliance.
- Provide feedback, in case of non-compliance, to the Nurse of the NSCSS SMH program.
f) provide crisis intervention, refer on an as needed basis to other services in consultation with the Nurse of the NSCSS SMH program.
g) co-ordinate with the Nurse of the NSCSS SMH program to provide:
- assistance to individuals connecting to direct care, satellite clinics and home videos, family and friends.
- connect clients with appropriate community programs, agencies or services based on identified needs.
- act as advocate for clients re: services and resources.
- on-going assessment of client status
- participate in case conferences as invited
h) cultivate a caring, supportive, non-threatening approach.
i) encourage the development of supportive networks
j) liaise with clients who are admitted to a local General Hospital and/or tertiary care hospital.
k) assess risk and make decisions to report to authorities without immediate available support (regarding suggestions of neglect, abuse, suicidal ideations, health and safety)
- 2. Community Relations:_
a) participate in public awareness projects.
b) ensure personal behaviour and communications reflect the philosophy and aims of the Program while on Program business in the community.
c) liaise with relevant organizations.
d) maintain effective working relationships with other service providers.
e) be current regarding services available.
f) act as Seniors’ Mental Health liaison to community committees or interest groups.
- 3. Professional Development:_
a) pursue activities that will ensure continuing professional growth and development (i.e. PIECES training; CPR, First Aid - see 6 D below)
b) share with co-workers knowledge and experience gained.
- 4. Administration:_
a) maintain current files on clients and respect their confidentiality.
b) collect, compile, and maintain statistics as required by the Ministry of Health/North East Local Health Integration Network, ADSAB and the Executive Director.
c) maintain electronic records (CRMS) of all client contacts as directed
d) adhere to the procedures regarding the financial systems in place
e) prepare reports as requested by the Executive Director or delegate
f) participate in the planning, procedure, policy, and evaluation process as requested.
g) document client interactions, goals, outcomes, and evaluation
h) adhere to policies and procedures of NSCSS
- 5. General:_
a) keep SMH program staff and the Executive Director informed regarding the program, clients, and your planned itinerary of activities (share calendar)
b) review incident reports regularly and recommend steps to prevent future incidents.
c) participate in scheduled staff meetings.
d) perform any other duties assigned by the Executive Director and/or delegate
