**Summary**:
Hero Home Care is seeking an experienced RN, LPN (or foreign trained nurse) to join our healthcare management team. As a Care Manager (RN or LPN) you will be supporting our North Shore, East Vancouver franchise group. You will be responsible for ensuring our clients receive a world-class home care and healthcare experience and will lead over 100 passionate caregivers as they make lives better in our community

**Duties & Responsibilities**:
The Care Manager will be supporting our North Shore & Vancouver territories. Responsible for ensuring our clients receive a world class home care and healthcare experience and will lead over 100 passionate caregivers as they make lives better in our community by doing the following:

- Lead meetings with new potential clients and family members in home and in hospital
- Manage a team of passionate Caregivers
- Design and implement care plans
- Liaise with hospital staff and community health workers
- Provide one-on-one training to caregivers
- Develop strong referral relationships in the community - sales management
- Collaborate with fellow management members to continually develop and implement best business practices

**Position Requirements**:

- 5+ years’ experience in the healthcare field
- Are a Registered Nurse (RN license) or Licensed Practical Nurse (LPN license) or have foreign trained RN experience of at least 3 years
- Have a valid driver’s license, access to a vehicle and comfortable driving throughout Vancouver
- Are comfortable using various CRM programs and smart devices
- Are flexible - available for normal as well as odd shifts in theevenings, early mornings and weekends
- Are self-managed - you set your own goals and hold yourself
- You love working in a team
- Are a Multitasker - you can wear many hats and manage time efficiently
- Are a Customer Service Fanatic - you will do whatever it takes to bring a smile to a face
- Are Determined - You will find a way to make it work
- Are Optimist - You can always find the positive in any situation
