Detailed Overview: Within the context of a client and family-centered care model and strengths-based, recovery-oriented practice, provides intake and transitional services for clients with substance use disorders requiring bed-based substance use and/or supportive housing referral services. Assesses referrals for eligibility for mental health and substance use bed-based services, matches referrals to appropriate bed-based programs, collaboratively develops individualized resource plans, and assists with transitions into permanent housing. Offers input into program planning and development and participates in meetings and committees, and participates in quality improvement activities, and promotes awareness of mental health and substance use programs and services. This position may be required to work at more than one location. Responsibilities:

- Follows evidence-informed best practices when supporting clients with desired outcomes; receives program referrals; assesses and screens clients based on established criteria; matches clients to appropriate bed-based mental health and substance use beds. Coordinates with provincial MHSU bed-based programs regarding screening and referrals to Fraser Health beds.
- Assists clients with linkages to vocational, therapeutic, health, social and education services. Recommends appropriate treatment and support services, and recommends recovery programs to a responsible clinician if a relapse occurs. Provides follow-up and promotes linkages with programs and support services.
- Works with responsible clinicians to support client access to services such as medical appointments, support recovery groups and social programs; provides healthcare information, contacts relevant programs/services, assists clients with processing forms, facilitates clients with transportation issues, and advocates for clients with other government agencies regarding referral and placement into bed-based substance use services.
- Provides comprehensive case consultation and support to multidisciplinary team(s) and other professionals from community services, withdrawal management, bed-based treatment contract providers, and other referral sources to support capacity-building, program development and continuity of care, based on evidence-informed best practice, regarding topics such as substance use treatment decisions, transition planning, crisis and safety issues. Supports the completion of pre-admission packages from referral sources. Ensures adherence to process algorithms for efficient admissions and reporting of vacancies, as a representative of Fraser Health, in coordination with the Manager/Coordinator.
- Prepares, maintains, records and charts information, and documents occurrences, referrals, contacts, and progress notes all in accordance with established policies. Maintains client records by methods such as documenting client assessments, interventions, evaluations, modifications and client goals. Prepares and maintains statistical data, correspondence, reports and other documentation as required.
- Manages waitlist and adheres to access and flow policies.
- Participates in relevant MHSU meetings, committee's community team meetings, continuing education, and conferences by methods such as gathering relevant information, providing input from a clinical perspective, presenting
