**Job Overview**:
The Outreach Case Worker manages a caseload of families, providing a variety of day-to-day support, including practical and social support, life skills assistance, and advocacy. You will work within a multi-disciplinary team and assist with building positive relationships within the community, including with families and other organizations. The Outreach Case Worker has a high level of independence in their schedule and responsibilities; strong communication and demonstrated reliability is required.
Hours of work are flexible and scheduled based on the needs of the families on your caseload. **However, you must be available to provide in-person outreach during the following hours: Mondays 1:45-4:30 pm & Wednesdays 8:15-4:30 pm.**
**Duties**:
- Client assessments: initial comprehensive assessments to understand client needs, challenges, safety concerns, and goals, and contribute to individualized service plans.
- Referrals to community resources when appropriate and follow-up with individuals and families to ensure they have successfully accessed needed services.
- Direct, community-based support to children, youth, and families both in and outside of their homes. Support includes but is not limited to practical life skills support (cooking, cleaning), transportation, attending appointments, etc.
- Crisis intervention and support: responding to emergent needs with compassion, supporting conflict resolution, and implementing crisis and safety plans.
- Administration and record keeping: maintaining client files and up-to-date records, progress reports, family check-ins, attending meetings, etc. Oversee clinical team documentation ensuring outreach workers are completing contact notes in a timely manner.
- Adherence to relevant privacy legislation (PHIA, FIPPA, etc.).
- Provide information to community members and service providers about programs and services.
- Facilitate training on special topics related to neurodiversity and mental health.
- Work collaboratively with team members and students; regularly attend and contribute to team meetings, clinical supervision, etc.
- Planning and facilitation of specialized individual and group programming.
**Qualifications**:
- Bachelors degree in Social Work, Psychology, Sociology, or related fields is preferred; other combinations of formal education, training, and/or lived experience will be considered.
- Prior experience in community outreach or social services, with a knowledge of available community resources.
- Strong interpersonal skills with the ability to build rapport and engage effectively with diverse populations.
- Ability to work independently and as part of a team.
- Strong leadership skills with the ability to motivate and support team members.
- Administrative skills including organization, attention to detail, and effective communication.
**Other requirements**:
- Valid class 5 driver's licence, access to reliable vehicle for work use, and insurance with adequate third party liability

Pay: $25.00-$35.00 per hour
