**Outreach Worker Job Description**
- _Summary: _ **Outreach staff are the key link to pregnant participants and new parents and families, the main**
“service-providers” of Healthy Start. The role is a paraprofessional / peer one that bridges the participant to professionals (e.g. nurses, dietitians, midwives, doctors) or to other community services. The position involves work with groups and individuals, as well as general liaison with the community.
**Reports to**: Program Coordinator **Teamwork**:The Outreach Worker works as part of the HSMM staff team to create and maintain a well-functioning workplace that is able
**Works closely with**: to focus clearly on program needs. The outreach worker does this program participants; other primarily by performing job responsibilities and by: outreach workers; both Program
Coordinators; site team members **Conducting themselves in ways consistent with the HSMM including childminders, volunteers, Respectful Workplace policy interpreters, facility staff, etc; ** Raising issues in an appropriate forum receptionist and data/office **Keeping internal HSMM issues well away from groups/ participants manager, ** Showing strong work ethic
- Contributing positively to staff and team discussions
- Sharing ideas for program and administrative improvements
- Participating in their own performance appraisal
**Responsibilities**:
1. Drop-in and **Prepares for and sets up the group, welcomes participants virtual group ** Does reminder calls to participants and new families/participants facilitation and **May drive a first-time participant to group support ** Ensures resources needed are available; carries needed items to drop-in
- Fills out forms as needed; distributes food coupons and bus tickets as appropriate
& according to current policy
- May assist in food preparation if needed
- Contributes to group session as facilitator of certain parts
- With other team members, attends to participants’ safety, social comfort and demonstrates a non-judgmental attitude
- Assists in clean-up
- Participates in debrief with team and takes notes
- Follows-up with details noted at debrief
- Liaises with, and supports volunteers, interpreters, childminders, etc. (may give direction if requested to do so)
- Attends and contributes to planning meetings
- Fills in at other sites as needed/requested
- Facilitate virtual groups in a similar style as in-person groups
2. Participant **Assisting families who indicate they want support outreach, ** Makes home visits (or other settings) as requested or identified support and **Listens to participants follow-up ** Assists participants in identifying their needs; supports, suggests and refers as
appropriate
- One-on-one contacts must have defined purpose; notes must be made of all
contacts
- May accompany (and drive) a participant to appointments such as doctor, court, social assistance, CFS, in cases of exceptional need
- Any driving of participants and children requires absolute compliance with seat belt and child car seat legislation and safe practice; no exceptions are permitted
- Works from a strength-based approach and endeavours to enable participants to find their own solutions and voices
- Consults with supervisor as needs, or as requested and always for high needs/high risk situations
3. External **Uses practical strategies to reach out to the focus population outreach and ** Regularly puts up program flyers in communities in which groups are located and promotion assists other staff as asked
- Liaises with other agencies; attends meetings as appropriate
- May be asked to do public speaking or man a display from time-to-time
- Cultivates awareness of work of relevant other agencies, to enable appropriate referrals and connections for participants
4. Paperwork / **Handles and completes forms required by funder (& Healthy Start), obtaining administration/ consent from participant first other tasks ** Keeps up-to-date files, notes and other records as directed
- Maintains confidentiality of participant files, but shares appropriately with other team members when needed
- Attends and contributes to Healthy Start staff meetings and planning meetings for group
- Assists in special events such us Making Baby Food
- Participates in general office chores on rotation with other staff
- Other tasks may arise from time-to-time
**Position boundaries and limits**
- The outreach job is not one of “counselling”. The role is paraprofessional and links participants to appropriate professionals and services.
- The outreach worker is friendly, but is not a friend.
- The outreach job is viewed as a position of trust to members of the community; job behaviours must take this into account and maintain appropriate professional and personal boundaries.
