Health Sciences North (HSN) is seeking an innovative leader for the position of Clinical Manager, Integrated Chronic Pain Program, OBIRS, CDSMP, Haven/Hemophilia, Cardiac & Pulmonary. The successful incumbent will provide leadership and direction through planning, organizing, and controlling of the day-to-day operations of the Integrated Chronic Pain Program, Outpatient Brain Injury Rehabilitation Service (OBIRS), Chronic Disease Self-Management Program (CDSMP), HAVEN/Hemophilia Program and Cardiac & Pulmonary Rehabilitation. **KEY FUNCTION**:
Provide leadership and direction through planning, organizing, and controlling of the day-to-day operations of the Integrated Chronic Pain Program, Outpatient Brain Injury Rehabilitation Service (OBIRS), Chronic Disease Self-Management Program (CDSMP), HAVEN/Hemophilia Program and Cardiac & Pulmonary Rehabilitation. **REPORTING**:
Under the general direction of the Administrative Director, Community Care and Rehabilitation/Regional Director, Regional Nephrology Program.

**DUTIES**:

- Design, promote, and facilitate the use of systems to ensure that the provision of patient care is consistent with program and professional standards.
- Liaise with Ministry of Health and Ontario Health/Local Health Integration Networks (LHIN) regarding program deliverables; adjust programming as required and provide requisite metric and financial reports.
- Develop and implement service delivery model that will allow Integrated Chronic Pain Program to expand across entire North East region
- Provide leadership support for Sudbury Outpatient Care (SOC) site and SOC leadership committee.
- Provide leadership support for Centre for Life (CFL) site including physical space logistics and partnerships with YMCA of Northeastern Ontario and City of Greater Sudbury.
- Provide management oversight and liaison with HSNRI of research activities across all programs.
- Participate in regional and provincial networks, expert panels and work/focus groups for each program within portfolio
