**Job Summary**

Services Coordinators.

This position is currently a hybrid model as our team is based at our provincial head office (Bayview/Eglinton); based on a 21-hour work week onsite a minimum of 2 days per week.

**Reporting to the Manager, Service Co-ordination & Intake you will be responsible for, but not limited to the following**:
Intake Services
- Referral networking, liaison, and outreach
- This position will provide support for provincial referral management
- Conduct client closure surveys.
- Provide an information and referral service for our community, support staff, referral sources and other stakeholders across the province.
- Provide back up Support as needed to all aspects of the client intake process provincially; determining service eligibility and assessing goals and interest in SCIO’s services.
- Support management of regional waitlists and liaise with staff, and referral sources.
- Input referrals into the case management system
- Maintain daily tracking and reporting mechanisms as required
- Contribute to the evaluation of client services data and cross organizational quality indicators specific to information and referral

Information Services
- Creates topic-specific FAQ blog content for the SCIO community
- Support Knowledge Exchange/Cortree research and education programs
- Contributes to the internal Wiki documenting FAQs
- Complete quarterly and annual statistical reports to meet client services funding requirements
- Actively participates on internal and external committees as deemed appropriate by the Director,

Knowledge Exchange
- Performs other duties and special projects as assigned by the Manager, Service Co-ordination & Intake

**Qualifications and Education**
- Demonstrated knowledge and/or lived experience with disability (spinal cord injury preferred)
- Community college diploma in social services worker, health promotion, rehabilitation services, or social science.
- Minimum two years' experience working and/or volunteering in a community resource capacity in the health or community service sector.
- Knowledge of community service/support networks
- Familiarity with accessible transportation services in the GTA and across the province
- Demonstrated commitment to continuous quality improvement and customer service standards in education, information and referrals
- Ability to work independently respecting professional boundaries in a team environment
- Proven computer proficiency working in an MS Office environment and data base management experience
- Willingness to travel as needed.
- Bilingual (English/French), an asset
