**The Opportunity**:
Reporting to the Manager of Patient Flow, the Patient Flow Coordinator is accountable for patient flow to support the Organization’s capacity to meet demand for service, with the goal of seamless, timely transitions through the continuum of care, ensuring that all patients are in the right place, at the right time, receiving the right care. In collaboration with the clinical team, patients, and families, the Patient Flow Coordinator is responsible for facilitating person-centered, collaborative discharge plans through comprehensive assessments, therapeutic intervention, and employing critical thinking and creative problem solving to support patient discharge. All responsibilities are expected to be carried out in accordance with the mission, vision and values of the MRHA.

**Qualifications**:

- Bachelor’s Degree in Health Care-related discipline, Master’s preferred
- Current valid registration and membership in good standing with the relevant Health-related College
- Minimum of 3 years of recent and related clinical experience, including discharge planning

**Competencies and Personal Attributes**:

- Demonstrated experience in the assessment and evaluation of the needs of patients to support individuals and families as they transition from and within a hospital setting
- Comprehensive knowledge of the impact of social determinants of health on discharge planning, Patient Flow and hospital system capacity
- Expert communication, collaboration, organizational, and problem solving skills
- Ability to work independently as well as within an interdisciplinary team
- Knowledge of local community resources
- Experience with legislation that governs Public Hospitals (Public Hospitals Act, Substitute Decisions Act, Consent to Treatment Act, etc.)
- Analytical skills to assess processes, identify barriers and develop comprehensive recommendations for improving patient flow; recognizing both internal and external resourcing and demands on services
- Ability to critically appraise literature to inform practice, including policy development
- Ability to complete projects to achieve agreed upon objectives and deliverables, and produce timely results within allocated resources
- Ability to work in a manner that is in compliance with patient/resident and employee safety practices, policies and procedures of the organization.
- Advanced computer skills, including Outlook, Word, Excel, PowerPoint, and EMARs (Cerner, Meditech)
- Ability to contribute to a work environment that is conducive to the organizations Workplace Violence and Harassment policy.
