**The Job Summary:
The primary purpose of this role is to ensure the highest standards of clinical care and data integrity within our facility. This involves managing and supporting the Resident Assessment Instrument (RAI) processes, including quality improvement and sustainability efforts, as well as overseeing the North American Classification of Rehabilitation and Surgery (NACRS) Lite processes and the Wait Time Information System (WTIS) Alternate Level of Care (ALC) reporting. The role requires a seasoned Registered Practical Nurse (RPN) or Registered Nurse (RN) with experience in Long Term Care and RAI MDS, who will provide leadership in clinical education, data quality auditing, and compliance with Ministry of Health and College of Nurses of Ontario standards. Additionally, this role includes administration of the Point Click Care system, development of individualized care plans, and maintenance of departmental documentation and educational materials.

**Key Accountabilities & Success Criteria:
- Supports RAI process, including maintenance, quality improvement and sustainability
- Supports the operational scheduling and completion of RAI MDS admission, quarterly, significant change in status, annual, and discharge assessments
- Manages and supports the NACRS Lite process, including maintenance, quality and sustainability
- Supports and monitors the WTIS ALC reporting process, including maintenance and quality auditing
- Supports ongoing leadership for education and mentoring to frontline staff
- Collaborates with Clinical Informatics Project Lead, actively participating in the development of clinical change initiatives
- Continuous proficiency in AIS, data quality, Ministry of Health Compliance as it pertains to clinical documentation and standards of documentation according to the College of Nurse of Ontario
- Data quality auditing and submission of records to the Canadian Institute for Health Information (CIHI)
- Point Click Care system administration including, but not limited to assigning login credentials, development of clinical documentation and management of system updates and releases
- Maintains current master copies of all departmental policy manuals, procedure manuals, Material Safety Data Sheets (MSDS), program manuals and education materials.

**Qualifications**:

- Successful completion of a recognized Registered Practical Nursing Program (RPN) or Registered Nursing Program (RN)
- Current certificate of competence in good standing from the College of Nurses of Ontario
- Minimum of 3 years clinical experience in Long Term Care is required
- Minimum of 2 years RAI MDS related experience required
- Current RAI Proficiency Certification (AIS) required
- In addition, certification, courses or other formal education on Post-Acute Complex Continuing Care MDS & Rehabilitation NRS is an asset

**Skills & Abilities**:

- Proven ability to get things done & demonstrated success leading new initiatives
- Capable of interpreting the data and reports to identify trends, issues and opportunities
- Exceptional communication (written and verbal), interpersonal skills (ability to develop a rapport with staff) and organizational skills (ability to prioritize work)
- Proficient Windows Operating System, Microsoft Office Suite
- Knowledge of adult education principles and demonstrated teaching/facilitation skills
- Evidence of good performance history and attendance record
- Experience with Point Click Care and creating individualized care plans strongly preferred
