**KIDS Team Service Coordinator**

The position is regularly 30 hours/week; from hire until March 31, 2025, 35 hours/week are available.
- Overview:_

The **KIDS Team Service Coordinator **will take direction from, and provide support to, the KIDS Team Lead. Duties will include direct service coordination for children and their families, as well as general office assistance in processing new referrals, and maintaining current files.

The KIDS Team Service Coordinator reports to the KIDS Team Lead and Executive Director.

Job Duties
- To attend KIDS Team meetings, as necessary
- To facilitate referrals for services outside of Chatham-Kent, as required.
- To collect discharge documentation from residential facilities, as necessary.
- To inform and involve families in the community-based plan of care process.
- To assist families in connecting to appropriate community service providers.
- To facilitate community meetings for children/youth waiting for services and updating service providers with relevant information.
- To update the KIDS Team Lead on the status of referrals.
- To prepare child/youth/families for a residential placement or out-patient services.
- To invite appropriate personnel to plan of care meetings/discharge meetings, and record minutes at the meetings.
- To facilitate discharge plans for children/families.
- To advocate for the youth and family in the community.
- To maintain files according to agency standards.
- To complete and provide monthly statistical reports as required.
- To provide coverage for the KIDS Team Lead, as necessary.
- To perform other related duties as required.
- Requirements:_
- Social Services Worker diploma or equivalent; preference will be given to BSW.
- 1-2 years of experience in a similar role would be an asset
- Demonstrated ability to respectfully assist clients, family members, and the community with care and concern, consistent with the Core Values, Vision, and Mission of Family Service Kent.

Pay: $24.29 per hour
