Here is what you will be doing:
The Sector Support, Performance and Accountability portfolio of Ontario Health focuses on high-impact provincial health system strategies, standards and initiatives aimed at providing information, evidence, and support to assess, inform and improve health system performance.

The Sector Support, Homecare and Community Services team will provide leadership and support for ensuring all patients receive accessible, high-quality homecare and community services. With deep content knowledge centered around delivery of homecare and community services, the team will collaborate across OH and with our partners (including Ministry and HCCSS) to provide support for this sector.

As the Manager, Sector Support, Homecare and Community Services within the Sector Support, Performance and Accountability Portfolio, you will be responsible for leading a high-performing team focused on providing support to all aspects of delivering care in the home and community, including community support services and assisted living.

This leader will have deep sector knowledge in home care and community services programs, services, and operations, including long-term care placement function.

- Here is what you will need to be successful:

- The Manager, Sector Support, Homecare and Community Services is responsible for overseeing the day-to-day operations and people management to deliver on key strategic initiatives, which include:
- Support the modernization of homecare delivery in Ontario through the Ontario Health team model-
- Plan, implement, deliver, and evaluate projects including monitoring risks and timelines.-
- Collaborate with key stakeholders in the determination of sector support needs and development of the team work plan and metrics-
- Support the procurement process, if a vendor for services or products is required-
- Monitor team capacity and provide access to resources to ensure the successful delivery of team’s mandate-
- Resolve issues identified by staff and escalate risks issues as required-
- Ability to identify opportunities for improvement- Operations and Administration- Participates in the departmental annual planning process and provides input to the overall operating plan with regards to - priority, resourcing, metrics-
- Identifies operational improvement opportunities by monitoring and measuring the portfolio’s performance-
- Monitor procurements and the performance of vendors-
- Implement OH policies and ensure compliance with portfolio-
- Resolves issues that are identified by staff and escalate when appropriate.- Human Resources- Oversight of team staff on a day-to-day basis including resourcing, workload distribution/management, quality assurance, and troubleshooting-
- Responsible for on-going coaching and support to the staff in relations to their Professional Development Plan-
- Responsible for the administration, development, and evaluation of the Performance Development Plan for all staff-
- Implements Performance Plan for staff not able to meet the goals and requirements in their PDP- Knowledge- Knowledge of key management accountabilities including planning, performance measurement, change management and people leadership-
- Deep content knowledge of the homecare and community services operating context and changing healthcare landscape-
- Quality improvement, project management and change management- Interpersonal and Communication Skills- Responsible for collaborating and maintaining relationships with key cross-portfolio contacts to ensure alignment with key external stakeholders.-
