**POSITION OVERVIEW**

The Program Manager oversees these service deliveries, supervising a team of Palliative Pain and Symptom Management Nurse Consultants and administrative staff.

**POSITION SPECIFIC RESPONSIBILITIES**
- Leadership/Management
- Ensure the alignment of programs with the Mission, Vision and Values of HWR
- Contribute within the management team in development of strategic directions and operational policies/procedures
- Management of Resources
- Responsible for the recruitment, orientation, training and development, mentoring, and performance management of all nurse consultants and administrative staff.
- Promote a safe working environment for all staff including health and safety awareness
- Clinical Consultation and Education
- Act in the Nurse Consultant role to care providers at all levels and for all sectors as workload permits.
- Serve as a mentor to inspire, develop, motivate and coach HPC team members and front line staff and leaders to deliver palliative approach to care
- Program Management
- Develop processes to build program capacity including communication links, resource kits and access to program resources
- Development of program and organizational metrics to support reporting to internal and external partners
- Responsible for provision of oversight to maintenance of service website
- Support to the Executive Director with the planning and execution of program strategic initiatives
- Management of program specific budget areas within HWR
- Compliance and Risk Management
- Develop and maintain a risk management framework for PPSMCS with a specific focus on policy and procedure and educational program content
- Internal External Relationship Management
- Liaise with and act in partnership and collaboration with other regional hospice programs and pain management programs
- Proactively seek and build relationships with allied organizations for program development and enhancement
- Participate at local, regional and provincial levels on relevant committees or projects with entities including but not limited to the Palliative Care Consultant Network, the College of Nurses of Ontario, Hospice Palliative Care Ontario, Registered Nurses Associaion of Ontario, Canadian Nurses Association, ON Health (OH)

**QUALIFICATIONS AND EXPERIENCE**
- Bachelors Degree in Nursing, current registration as a Registered Nurse with College of Nurses of Ontario
- Canadian Nursing Certification in Hospice Palliative Care
- Previous experience in supervision and management of programs and staff
- Detailed understanding of the health care system including OH Teams, Home & Community Care Support Services (H&CCSS), Local Health Integration Networks, acute care hospitals, community care, long term care, primary care and community support services

**SKILLS AND ATTRIBUTES**
- Excellent communication skills including courtesy, tact, discretion, providing detailed explanations, interpretation and diplomacy.
- Advanced assessment and clinical reasoning skills
- Technically proficient with MS Office products and specialty software (such as D2L Brightspace)
- Excellent planning, organizational and project management skills
- Ability to adapt to changing requirements and environment.
- Ability to manage time constraints and quality pressures.
- Self-motivated, committed, very organized and sensitive to the needs of clinicians and staff.

- Demonstrate commitment to continuously improving professional excellence.
- Ensure awareness and maintenance of best practices and standards for all HWR activities.
- Develop and maintain positive relationships with partners and the wider community, as appropriate.
- Mentor, teach and encourage a collegial atmosphere for all staff.
- Incorporate and strengthen a team based working environment that empowers staff to be collectively responsible for ensuring HWR achieves its mission.
