**GENERAL DESCRIPTION**:Community PEERS is a community-based program developed in response to youth violence prevention support requested from community stakeholders and frontline workers across Toronto. This position reports to the Senior Manager, Youth and Justice Services and is part of **CUPE 2289.**
**MAJOR DUTIES**:

- Coordinate the hiring and training of Peer Support Workers on a bi-annual basis
- Work closely with the assigned Community Development Officer (CDO) at the City of Toronto to receive triaged/approved service requests
- Coordinate and assign Peer Support Workers to service requests in a timely manner (within 48-72 hours)
- Maintain, track and updates service requests database regularly
- Support Peer Support Workers with program development, one-on-one session planning, and referral processes, as required
- Onsite support for community engagement activities, service requests, and as required
- Flexible evenings and weekends
- Provide pre-brief and de-brief supports to Peer Support Workers
- Follow-up with Peer Support Workers on required administrative duties (program plans, summary reports, incident reports, etc.)
- Provide immediate support and direction to Peer Support Workers in crisis/critical incident scenarios
- Meet with assigned CDO on a bi-weekly or monthly basis to share updates, project development, areas for improvement, challenges, etc.
- Lead the feedback and evaluation process for Peer Support Workers
- Refer Peer Support Workers to existing community, TNG, and City of Toronto resources for additional supports, as needed
- Lead ongoing Community of Practice sessions and other engagement activities for Peer Support Workers to encourage ongoing learning and development, case support, networking, and program development, etc.
- Manage program stakeholder and partnerships, including: outreach to community agencies across the City of Toronto, community service requests and referrals, Community PEERS PSWs placements, and development of Memorandum of

Understandings (MOUs)
- Compiles and submits interim and final reports to the City of Toronto as needed
- Provide overall oversight and coordination of the PEERS Program budget
- Development and maintenance of payment tracking tool to track completed tasks and payment of Peer Support Workers
- Submit payment tracking tool to TNG’s finance on a weekly basis to ensure ongoing and timely processing of payments for Peer Support Workers
- Ensure Community PEERS Peer Support Workers are accessing training opportunities through the Community PEERS program
- Work with assigned CDO and evaluators to support program evaluation processes

**REQUIREMENTS & QUALIFICATIONS**:

- B.S.W or B.A. in related field and two years’ recent experience in case management, placement and support of young persons OR a combined equivalent of education and experience
- Knowledge of and experience working within the Youth Sector
- Proven ability in providing individual support, protocol and documentation practices
- Ability to develop and maintain partnerships with relevant youth community resources
- Ability to work remotely and to travel between service locations in the City of Toronto
- Proven training and experience in conflict prevention strategies, mediation, and stress management an asset.
- Excellent verbal and written communication and presentation skills in English, second language an asset
- Service knowledge of the City of Toronto
