Position Summary

KEY DUTIES & RESPONSIBILITIES

Liaise with the Placement Coordinator at HCCSS to notify of bed vacancies, mange the internal and external waitlist obligations, and ensure bed occupancy and census targets are maintained within Rideaucrest Home.

Pre-placement visits and planning for new admissions will support the new resident, their loved ones, and the care team to ensure needs are understood in advance of admission day, and this role will advise on pre-admission care planning in the Home to support a successful transition to long-term care.

Once the resident/family have accepted a bed at Rideaucrest Home, the Supervisor, Resident Admissions & Experience will oversee the administration admission processes, guide the team on rate reductions, census accuracy, financial supports, intricacies of decision-making authority and decision-making capacity.

Coordination and facilitation of admission and annual Care Conferences for all residents as outlined in the FLTCA, 2021. Ensuring that members of the interdisciplinary team are scheduled in advance for care conferences, allowing for the team to gather information relevant to the resident’s care in advance. Ensuring that residents and families are invited to attend the conference in a way that suits their needs and accessibility; in-person, virtual, telephone.

Complete facility tours for potential residents and/or families, addressing inquiries on the care and services provided at Rideaucrest Home. This is essential in sharing the Home’s competitive advantage in care services, while maintaining an adequate wait list to ensure provincial occupancy targets are maintained with future admissions.

Leadership contact, with support administrative clerical team, for ongoing communication with residents and families related to advising on care and services in the Home and connecting residents with opportunities for community resources and engagement.

Qualifications, Competencies
Allied Health Professional with Degree/diploma in recreation and leisure studies, therapeutic recreation, kinesiology or a related field

Allied Health Professional with Degree/diploma in therapeutic recreation preferred

5 years experience in a health care or long-term care setting

5 years experience in a leadership capacity

Certification in Gerontology preferred

Gentle Persuasion Approach (GPA) certified preferred

**Must demonstrate corporate competencies**: Customer Focus, Results Orientation, Integrity, and Teamwork
Skills, Abilities, Work Demands
Knowledge of the FLTCA, 2021 and its Regulations, Occupational Health & Safety Act and related policies, procedures

Working knowledge of Health Partner Gateway (HPG)

Working knowledge of Point Click Care (PCC) preferred

Knowledge of community, resources, programs and services, and issues pertaining to older adults, including knowledge of human behaviour and counselling techniques

Able to work in a cooperative manner with all members of families, residents, staff, and community partners

Skills in gerontology, with the ability to focus on special care needs with empathy, respect, and a supportive approach

Advanced proficiency in Microsoft Office

Excellent verbal and written communication skills

Strong, effective customer service and interpersonal skills

Strong and effective problem solving skills

Proof of recent screening for Tuberculosis (TB)

Proof of immunity to communicable diseases

Proof of full COVID-19 immunization receipt (2 doses)

Required to obtain and maintain a satisfactory criminal record check, including vulnerable sector.
