**Position Title**: Care Coordinator

**Status**:FT Support

**Hours**: 37.5/hrs - Hybrid; some days in-person and some days remote. Working hours are 8:30 am to 5:00 pm, 4 days/week with an in-person evening shift of 10:00 am to 6:30 pm 1 day/week. Opportunity to explore a compressed work week.

**About the Care Coordinator** **Role**:
Reporting to the Manager, Student Connection and Community Care, the Care Coordinator provides non-clinical case management and system navigation support to enable student success and wellbeing.

Incumbents provide on-going non-clinical case management to learners, as well as guidance and support to faculty and staff to facilitate the resolution of complex issues (e.g., mental health, suicide ideation, homelessness, abuse, food insecurity, sexual and gender-based violence, etc.) to support learner success at Humber Polytechnic and the University of Guelph-Humber. Incumbents also develop and maintain relationships with various partners in the community in order to provide holistic support to learner success.

The incumbent is an active member of Humber Polytechnic’s CARE team (Coordinate, Assess, Respond, and Educate), an interdisciplinary team that meets regularly and provides institution-wide response to addressing challenging and complex issues and critical student behaviour. Functions supporting CARE include: gathering relevant case information, communicating and liaising with faculties and other departments about specific students of concern, responding to initial inquiries about supporting students who may be demonstrating concerning behaviour, and formulating case planning and management with students. The Care Coordinator will also be the primary intake for all student disclosures of sexual violence. This includes connecting with the survivor and/or respondent to triage appropriate services both on and off-campus. The incumbent is responsible for maintaining highly sensitive and confidential databases and documentation to ensure proper record keeping, file retention, and process is being followed.

**Education**
- A Bachelor’s degree (4 year) or equivalent combination of education and experience. Degrees in areas such as education, psychology, social work, criminology, or programs emphasizing human development are an asset.
- Certifications in SafeTALK and/or Mental Health First Aid, and ASIST are required.
- Anti-oppressive and trauma-informed mental health and/or sexual violence awareness training considered an asset.
- Case management certification and/or courses considered an asset.

**Experience/Skills**:

- Minimum of 3 years’ experience in a post-secondary environment
- Knowledge of, and demonstrated experience with, non-clinical case management strategies, including supporting individuals with complex needs (behaviour management, mental health, food insecurity, homelessness, academic concerns, etc.)
- Knowledge of, and demonstrated experience in, intersectional, feminist, anti-oppressive, and anti-racist practices relating to sexual violence, intimate partner violence, and family violence.
- Excellent knowledge of survivor-centered and trauma-informed professional practice.
- Excellent conflict resolution, interpersonal, client relationship, and communication skills (oral, written, and presentation).
- Ability to engage and build rapport with various stakeholders and members of the institution’s community.
- Good knowledge of principles and practices of administration in a post-secondary educational environment.
- Ability to work in a team setting and independently.
