Full-time Day Program Support for Adults with

The Centre for Opportunities, Respect, and Empowerment (CORE) provides community participation and day program supports for adults with developmental disabilities and dual diagnosis. The program provides participants with the opportunity to develop life skills and to engage in social, recreational, and personally empowering experiences. CORE is a respected non-profit organization in the developmental sector, funded by the Ministry of Children, Community and Social Services, and by CORE’s not-for-profit fee-for-service program spaces.
CORE has an immediate opening for a Lifestyle Strategist - Level 2. This opportunity is great for someone looking to build their skills and experience with the developmental services sector in a frontline position. The individual will be responsible for direct service delivery to CORE participants and will use energy and creativity to assist with activity facilitation and day-to-day operations.
**Working Environment**
- Full-time position with permanent employment after successful completion of a 6 month probation period

**General Responsibilities**:
- Supporting with day-to-day operations
- Assisting with activity facilitation
- Demonstrating value of all participants
- Communicating with the staff team
- Participating actively in staff meetings

- Presenting and conducting self in a professional manner
**Lifestyle Strategist - Level 2 is responsible for**:
- Familiarizing self with individual participant needs and goals
- Developing and implementing engaging activities based on participant goals
- Providing emotional and behavioural support
- Seeking and utilizing supervision appropriately
- Writing meaningful and accurate case notes as required
- Demonstrating effective written and verbal communication
- Managing a caseload of 15-20 program participants
- Facilitating participant skill development
- Facilitating meaningful and safe community outings
- Encouraging participant peer relationships
- Using de-escalation techniques to prevent crisis situations
- Identifying gaps in service offerings and creatively problem solving to meet participant needs
- Developing keen knowledge of and relationships with individual participants
- Drawing from established knowledge of participants when facilitating goal setting
- Demonstrating investment and value in supporting participants throughout the process of goal attainment and celebrating accomplishments
- Developing Individual Support Plans and Annual Progress Review documents as scheduled
- Ensuring a high level of quality, accuracy, and meaningfulness in all documentation
- Ensuring that participant files are kept up to date and tidy
- Demonstrating effective written and verbal communication with external stakeholders
- Leading Annual Progress Review meetings for individuals on caseload
- Advocating on behalf of participants
- Demonstrating good judgement, decision making, and problem solving
- Offering feedback and acting as a resource to peers
**Reporting Relationships**:
- Reports directly to the Program Manager
- Works in collaboration with all other team members
**Knowledge**
- Minimum of a college diploma in a relevant human services field of study
- Understanding of the social service field and vulnerable populations
- Ability to communicate with a variety of stakeholders in a professional manner
- Strong writing and documentation skills
- Skills in developing and facilitating engaging groups and activities based on individual goals
**Experience**
- Minimum of 1 year experience in supervised related field
**Equipment**
- **Hardware**:computer, telephone, photocopier, printer, scanner, shredder
- **Safety Equipment**:personal safety whistle, gloves as needed, protective equipment for common cleaning chemicals, COVID-19 person protective equipment
**Benefits and Incentives**:
- Paid vacation entitlement is five days after completion of each six month period. Additional vacation days are earned after 5 years of seniority, as per CORE’s Vacation policy. Vacation scheduling is on an availability basis.
- Paid sick time is earned at one day per month
- Eligibility for group health benefit plan upon completion of probation period
- May join CORE’s group RRSP plan to make voluntary payroll contributions, and eligibility for group RRSP employer contribution one year after completion of probation period
