Program Coordinator for Adult Day Programs

Position Type:

Temporary Pro-rated (up to 5 months)

Hourly Pay Rate:

$26.05 - $26.72


Bi-Weekly Hours:

60 Hours


Hours of Work:

7.5 hrs/shift


The Baycrest Adult Day Program focuses on providing older adults with a wide range of social, cognitive, physical, and spiritual activities. This program is designed for community-dwelling seniors with physical frailty as well as moderate to severe cognitive impairments.

Responsibilities include:

  • Providing safe and effective direct care and support during programs to meet the needs of program members as identified in the interdisciplinary care plan
  • SUPPORTING THE INDEPENDENCE, DIGNITY, SOCIAL, EMOTIONAL AND CULTURAL WELL-BEING OF PROGRAM MEMBERS
  • Promoting an environment conducive to dementia care, mental health care, and physical frailty in a community day program setting
  • Supporting the delivery of in-person and virtual programs related to health and wellness of program members
  • Planning and conducting assigned programs as well as initiating new programs and planning of special events under the guidance of Therapeutic Recreation
  • Supporting members being transported to and from the program while on the Baycrest bus
  • Initiating and maintaining therapeutic relationships with program members, their family, and other care providers

Qualifications include:

  • Current P.S.W. certificate from a recognized community college or educational institute or equivalent combination of education and experience
  • Experience in program activation, recreation, and virtual program required
  • Activation Diploma preferred
  • Current Basic Cardiac Life Support (BCLS) required
  • Certificate in Dementia Care preferred
  • Ability to work independently and as part of the interdisciplinary team, effectively managing challenging situations including the behavioural and psychological symptoms of dementia
  • Excellent interpersonal, communication, organizational, and problem-solving skills
  • Experience working with at-risk or frail seniors living in the community preferred
  • Experience delivering services and care to seniors with cognitive impairment, mental health issues, and complex medical issues
  • Experience working in a Day Centre or community service setting preferred
  • Knowledge of Yiddish, Hebrew, Russian languages an asset
Salary:

$52,104 - $54,096 per annum
