Reporting to the Manager, Patient Safety and Risk, the **Patient Safety and Risk Advisor** is responsible for leading and supporting system-wide and organizational efforts to ensure patient safety. Specifically, this role focuses on ensuring that patient care is provided in a safe manner for patients and care providers, and that professional and evidence based practices are utilized by the organization in a patient-centred manner. This includes working in partnership with health service providers and the organization’s contracted service provider organizations to developed shared approaches and strategies for patient safety. Leadership in key areas of enterprise clinical risk management, patient safety, ethical decision making, patient relations, and emergency management will be essential in this role.
The Patient Safety and Risk Advisor will demonstrate an understanding of quality, risk and patient safety principles and practices, following all safe practices and procedures to support a safe patient and working environment.
This is a temporary full time position in our London, St Thomas, Stratford, Woodstock or Owen Sound locations with an anticipated duration of twelve (12) months.
What will you do?
- Supports the development and implementation the organization’s patient safety goals through projects and initiatives, in accordance with strategic plan of the organization, provincial health legislation and regulations, and accreditation standards.
- Participates in the planning and preparation with the organization’s Accreditation cycle.
- Supports the development of a patient safety culture within the HCCSS that promotes safety in reporting and focuses on learning and improvement.
- Supports the planning, design, implementation, evaluation and documentation of the organization’s patient safety activities, including Patient Safety Risk Assessments and Patient Contingency Plans
- In partnership with other quality team advisors responsible for leading, planning, directing and coordinating individual improvement initiatives focused on meeting quality and patient safety best practices and standards.
- Supports the review, analysis and debrief of key patient safety events (adverse events) to understand root cause and to implement improvement activities.
- Supports the development and execution of the HCCSS South West’s Wound Care Framework and Falls Prevention Strategy
- Supports the Patient Relations Advisor within the organization in managing and responding to clinical risk events.
- Consults on cases and where necessary, assists staff to handle and resolve patient safety issues and complaints in a timely and effective manner and involves Patient Care Managers as appropriate when required for coaching and support of staff
- Effectively establishes and maintains collaborative working relationships with teams to achieve engagement and participation in patient safety initiatives.
- Executes quality and risk management initiatives at the patient, system and sub-region level to identify challenges and solutions to improve health system outcomes.
- Supports the Manager, Patient Safety and Risk in communicating with and advising the Leadership Team on matters related to patient safety planning, outcomes, issues management and legal and other requirements.
- Participates in provincial working groups, committees and other initiatives to advance shared objectives, develop best practices and/or align efforts across the health system.
- Partners with other HCCSS leaders and staff to cooperate across functions to deliver and align projects, initiatives or services.
- Participates in and contributes to provincial inter-HCCSS planning and engagement activities, as required.
What must you have?
- University undergraduate degree in nursing, health administration, public administration or related field.
- Greater than 2 years of experience specific to patient safety, quality improvement, performance management, and risk management in a healthcare environment or equivalent combination of education and experience;
- Demonstrated change management, facilitation & coaching skills
- Communication and inter-personal skills, to work effectively and develop trust with internal and external colleagues, and to liaise with the Ministry of Health and Long-Term Care and other provincial partners.
- In-depth knowledge of patient safety principles, methodologies, and statistical and analytical processes and techniques
- Current knowledge of legislation relevant to patient care and healthcare in Ontario (e.g. ECFFA etc.)
- Current knowledge of Accreditation Canada standards
- In-depth knowledge of quality improvement tools, techniques, research, theories, trends, and measur
