The **Mental Health and Addictions Case Management Program** assists people who are having significant difficulties in
day-to-day living as a result of severe and persistent mental health issues or mental health and substance use
challenges. Services are provided in English, Tamil, and several other languages.

**What You Will Do:
Supervision:_ _
- Provide and coordinate direct supervision and support to 11-13 staff and participate in indirect leadership with

Health Services colleagues
- Review client caseloads with staff to ensure client progress; access to services/resources; service co-ordination;
and timely discharge to services/resources
- Review and monitor client files regularly and provide feedback to staff
- Review the quality and timeliness of client records, documentation and standardized tools such as the Ontario

Common Assessment of Need (OCAN)
- Ensure the completion of Client Crisis and Emergency Plans
- Ensure the completeness and accuracy of client records
- Support staff to develop Coordinated Care Plans (CCPs)
- Ensure Funder targets are met
- Provide and facilitate regular performance reviews for all direct reports
- Ensure that staff have access to all relevant training opportunities

Administration/Organizational

- Attend and participate in supervision meetings with the Senior Manager
- Approve monthly expenses for staff
- Approve bi-weekly staff timecards
- As required, complete operational statistical reports
- Participate in budget development and management
- Participate in ongoing evaluation, development and review of the Program
- Participate in the Ontario Perception of Care (OPOC) process and assist in the participation of client feedback
- Participate in the development and writing of operational policies and procedures
- Recruit, hire and train staff
- Participate in the development and writing of program policies
- Attend and participate in organizational events
- Participate in relevant internal/external committees
- Support and monitor Health and Safety requirements, ensuring a safe work environment for staff
- Ensure compliance to Fred Victor’s Policies
- Ensure compliance to Health Services Program Policies
- Other duties as assigned by the Senior Manager and/or Director

Leadership:

- Develop and foster a constructive, dynamic and cohesive team and work with team to develop and implement

effective strategies to serve clients

- Support and encourage successful relationships between all partner agencies
- In partnership with the Senior Manager, outreach and develop links with system partners
- Participate in relevant presentations and workshops
- Participate in potential funding proposals
- Contribute to the achievement of Fred Victor’s Strategic Plan

**What You Bring to the Team (Qualifications)**:

- Degree/Diploma in Social Work, Social Sciences, Health or related area
- At least 5 years mental health and addictions case management experience working with people experiencing

severe and persistent mental health issues or mental health and substance use challenges
- Demonstrated experience in leadership roles, direct management of frontline teams is an asset
- Knowledge and understanding of major mental health diagnoses
- Knowledge and understanding of case management roles and functions
- Demonstrated experience working in a client-centred approach in case management
- Knowledge and understanding of trauma informed practice
- Knowledge and understanding of working with clients with substance use issues, and knowledge and
