Overview

The MHW - Case Manager collaborates with patients and their families to provide individualized and flexible services to engage, support, and integrate patients to the community, and facilitate their use of resources and supports to maximize their independence and potential. The collaboration will result in the comprehensive, patient-centered, sustainable discharge plan from hospital to the next destination of care. As part of this process individuals are assisted to acquire and use skills, and to connect with formal and informal supports, peer support, professional services, and resources to promote their recovery, improve their quality of life, and reduce the likelihood of relapse.

The MHW - Case Manager will begin the case management and clinicalcare coordination process by completing a comprehensive psychosocial assessment and identification of internal and external supports; understanding of patients’ strengths and challenges; patients’ and families’ wishes as well as risks and barriers for timely discharge, and appropriateness and availability of community recourses. The MHW - Case Manager will liaise with the other members of interprofessional team, while utilizing their knowledge of community resources and applicable legislation.

Education
- Bachelors degree in health or social services required, BSW preferred.
- Registration in good standing with relevant professional college.

**Qualifications**:

- Must have at least 2 years’ recent relevant experience in community-based mental health settings. Experience in mental health inpatient settings is considered an asset.
- Working knowledge of supports and MH services in community including formal andinformal resources is required
- Knowledge of systemic issues such as poverty, unemployment, stigma, medication adherence and the isolation felt by individuals with serious mental health issues and their families is required
- Experience working with individuals with addictions preferred
- Demonstrated ability of case management skills is required, demonstrated ability of crisis interventions skills is considered an asset
- Demonstrated understanding of the principles of psychosocial recovery,using a client-centred

approach.
- Experience working with acute care mental health population is required, experience working with the psycho-geriatric population is considered an asset
- Demonstrated knowledge, skills and ability to complete psychosocial assessments using various sources of information.
- Demonstrated knowledge and skills in discharge planning, conflict resolution and creative problem solving required.
- Demonstrated working knowledge of legislation including the Mental Health Act, PHIPA requirements, Substitute Decisions Act, Health Care Consent Act, Alternative Level of Care, Public Hospitals Act.
- Demonstrated ability to prioritize multiple competing workload demands.
- Superior communication and negotiation skills.
- Demonstrated ability to provide resource identification, referral and linking.
- Demonstrated ability to work independently and as part of an interdisciplinary treatment team.
- Demonstrated ability to build relationships with internal and external partners.
- Demonstrated ability to prioritize and work effectively in a fast paced environment required.
- Demonstrated patient centered philosophy of care provision.
- Self-directed, motivated and energetic.
- Good work and attendance record required
