**Support**:To provide support based on the person’s service plan.
- Support people in all aspects of life including their home, work, leisure and other community involvement in a respectful and creative way.
- Work in a manner that is conscientious, respectful and supportive of people’s desire to live their best lives.
- Support the development of skills related to personal care, homemaking, personal and home safety, personal wellness, healthy positive relationships, budgeting and any other identified areas.
- Recognize and celebrate accomplishments with people in their lives.
**Empower**:To assist the person in achieving their personal goals.
- Participate in the development of a person’s life plan, support plan and support agreement.
- Share ideas on how to achieve plans and goals and foster opportunities to achieve these.
- Foster self-advocacy skills and act as an advocate when needed while endeavoring to overcome challenges and barriers.
- To support increased opportunities for meaningful connection and belonging.
**Community**: To assist the person in the development of meaningful ties to their community, through active involvements.
- Support the person’s development of meaningful and fulfilling personal and community connections through participation.
- Creatively explore opportunities to enjoy a variety of experiences.
- Assist the person in locating and participating in personal and social activities and developing natural relationships.
**Organizational**: To be committed to the mission, principles and goals of the organization and adhere to the policies and procedures.
- Respect and uphold confidentiality at all times and represent the agency in a positive and professional manner.
- Demonstrate respect for and celebrate the uniqueness of each person at all times.
- Identify and actively participate in personal training and development opportunities through performance reviews at least annually.
- Maintain agency property to acceptable standards.
- Keep informed of organizational information and attend meetings as required.
- Work effectively as part of the person’s support team in a manner that demonstrates Unconditional Positive Regard for all members.
- Maintain relevant, respectful, complete and accurate records for people accessing support and complete reports as required.
- Follow appropriate standards and procedures of health and safety related to: medications, medical care and treatments, transportation, assistive devices and safety - home and personal
**Successful applicants will hold the following qualifications**:
- Experience in supporting people with disabilities in community living is an asset.
- Demonstrated ability to work as part of a team, encouraging unity and a positive, progressive atmosphere.
- Current First Aid/CPR and Crisis Intervention (CPI) certificates or a willingness to obtain this.
- The ability to act as an advocate for individuals in a manner that fosters individual growth and change.
- Valid Class G Driver's License
- Vulnerable Sector Criminal Reference Check current within the past year, or a willingness to obtain this
